The application information includes Mrs. Roosevelt's date of birth (10-11-1884), sex (Female), weight (170 lbs.), height (5 Ft. 10 In.) eye color (Blue), and hair color (Gray). There is also a section to record health issues in which is it noted Mrs. Roosevelt has: slightly defective hearing no change. The form is not dated or validated, but is signed in ink on the reverse: Anna Eleanor Roosevelt.
