DescriptionA two-part (still connected) New York State Bureau of Motor Vehicles Operator's Renewal Application for licenses expiring September 30, 1958.

The application form (the left half), for the renewal of license number 12571, is made out to: Anna Eleanor Roosevelt / Hyde Park / Dutchess NY. The application information includes Mrs. Roosevelt's date of birth (10-11-1884), sex (Female), weight (170 lbs.), height (5 Ft. 10 In.) eye color (Blue), and hair color (Gray). There is also a section to record health issues in which is it noted Mrs. Roosevelt has: slightly defective hearing no change. The form is not dated or validated, but is signed in ink on the reverse: Anna Eleanor Roosevelt.

The right half of the form is the Operator's License, good until September 30, 1961. It is signed on the front: Anna Eleanor Roosevelt.
