Physical DescriptionA red, white, and blue window decal form for the U.S. Office of Civilian Defense (OCD). At the center of the white and red background is the insignia of the U.S. Office of Civilian Defense and a large red check mark. Blue text above the insignia reads: AIR RAID PRECAUTIONS HAVE BEEN TAKEN HERE. Below the insignia is a "Signed by" signature line and an address line to be completed by a Warden. A strip of dry adhesive runs along the front top of the decal.
