There are an additional two larger levers on the left side and a red reset lever on the top. The manufacturer's plaque is mounted at the bottom center.

The machine includes paper inserts for the Republican and Democratic party candidates and ballot questions for the 1936 presidential election in Pennsylvania. The candidates listed are Franklin D. Roosevelt and John N. Garner for President and Vice President on the Democratic ticket, and Alfred M. Landon and Frank M. Knox for President and Vice President on the Republican ticket. There are also voting choices for five ballot questions, as well as Commonwealth of Pennsylvania Democratic and Republican voting choices for Governor, Lieutenant Governor, Comptroller, Attorney General, Chief Judge of the Court of Appeals, United States Senator, and Representatives in Congress.

Historical NoteThis type of machine was used to instruct voters on how to vote on the larger, actual "gear and lever" or "automatic" voting machines.
